The name change process
- Complete your deed poll application on this website.
- Review and sign your deed poll document.
- Start notifying organisations — HM Passport Office, DVLA, banks, employers, and others.
- Update your records with each organisation using your deed poll as evidence.
Who to notify
After receiving your deed poll, you should notify the following organisations. Always send a certified copy — never your original document.
- HM Passport Office — to update your passport
- DVLA — to update your driving licence and vehicle registration
- HM Revenue & Customs — to update your tax records and National Insurance
- Your bank and financial institutions
- Your employer and HR department
- NHS (GP, dentist, and hospital records)
- Electoral register (your local council)
- Utility providers and subscription services
